Application Form

All applicants must submit an official application form online at the U of C Graduate Studies site.

Applications must be accompanied by a non-refundable fee of $100CAD for Canadian citizens and permanent residents, and $130CAD for International students. Please follow all instructions carefully, and note that references must be on the prescribed forms.

Supporting documentation (for example: official transcripts, references [must be on the prescribed U of Calgary forms to be acceptable], fee receipt) to complete your application should be mailed to:
